is Person/Family-Centered Planning?
Person/Family-Centered
Planning is a process provided to assist an individual
in identifying and planning for his/her present and future
dreams and desires. Person/Family-Centered Planning is
provided to all consumers receiving mental health services.
Who
can benefit from Person/Family-Centered Planning?
Anyone
who has a mental illness, developmental disability or
serious emotional disturbance may benefit from Person/Family-Centered
Planning.
How
does Person Centered Planning help an individual?
Person/Family-Centered
Planning is a way of helping an individual. It depends
upon the relationship and mutual trust between the individual
and those who are helping to plan his or her care. The
individual chooses those who will help with Person/Family-Centered
Planning. These people help the individual identify personal
choices and gain more control over his or her life. These
decisions may include where to live; how to spend time;
vocational training; health and safety; employment; and
others. The individual's wants and needs are accommodated
as much as possible to assure successful planning.

can I participate in the Person Centered Planning Process?
All
programs of Bay-Arenac Community Mental Health use a Person/Family-Centered
