What
are Case Management Services?
Case
Management helps people gain access to services they need
to be independent in the community. These services can be
medical, social, or educational. They may also include behavioral
health services, vocational services, or others. Case Management
is usually provided within the home, community or vocational
setting. It varies in intensity based on the needs of the
individual being served.
Who
can benefit from Case Management Services?
Anyone
who has a severe or persistent mental illness or serious
emotional disturbance and meets criteria for mental health
services may be eligible for Case Management Services. Services
are based on the needs of the individual.
How
do Case Management Services help an individual?
A Case
Manager helps an individual to more fully participate in
his or her community. A person-centered planning approach
is used to help identify an individual's strengths, dreams,
goals and desires. The Case Manager may also help in other
ways. These may include: finding the best place for the
individual to live; encouraging him or her to achieve a
higher level of functioning and independence; assisting
with coordination of assessments, monitoring, advocacy,
liaison with inpatient settings, crisis intervention and
other services; and referring the individual to the appropriate
services required.
