Case Management Services

Case Management Services include assessment, treatment planning, advocacy, linking and coordinating with community resources and monitoring of services.

• Helps people gain access to needed community services and resources in the areas of housing, education, medical/health care, behavioral health, financial, social/ recreational, transportation and vocational services.
• Helps coordinate services when the individual is involved with multiple agencies and/or programs
• Coordinates with the psychiatrist when an individual is receiving medication services to treat a mental illness.
• Provides supportive services such as social skills training, parent education, help learning new coping skills, development of new ways of relating, supportive counseling, and crisis intervention,

Case Management Services are often provided in the individual's home or in other community settings. Frequency of contact depends on the needs of each individual.

Adults of all ages who have a serious mental illness and who need assistance with accessing and coordinating multiple services may benefit from Case Management Services. Persons who need help staying on their psychiatric medications and managing their mental illness may also benefit from Case Management Services.

Case Management Services helps people to manage their mental illness, access community resources, participate in the community and helps them to be as independent as possible. A person-centered planning approach is used to help identify an individuals strengths, dreams, goals and desires.
